Insurance and Billing
At Great Lakes Jaw and Implant Surgery Center we make every effort to provide you with the finest surgical care and the most convenient financial options. To accomplish this goal, we work with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. Please bring your insurance information to your first office visit.
Remember, insurance contracts are agreements between patients and their insurance companies. The coverage that you receive is based on the terms that have been negotiated for your particular plan. Policy plans differ based on such terms. For that reason, Great Lakes Jaw and Implant Surgery Center cannot be responsible to know all the coverage benefits for any specific plan. We do bill insurance claims for you, but your insurance may or may not cover the procedure(s) planned for you in this office. It is the patient's responsibility to be aware of contract benefits and limitations.
For your convenience, we accept Visa, MasterCard, Discover, personal checks, or cash. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the the time service is rendered unless other arrangements have been made in advance.
We also work with Care Credit, a healthcare financing company you can enroll with online for extended credit to help you pay for procedures.

Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We will send you a statement after we have received payment from your insurance company. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect payments made by your insurance company during that time period. Any remaining balance after your insurance has paid its portion is your responsibility. Your prompt payment is appreciated. In certain circumstances, we can make arrangements for a monthly payment plan, but that must be implemented prior to any actual procedures.
